Reminder: In order to participate in the athletic programs at the Woodbury Middle School, You must have returned the three forms for eligibility:

1. A school physical as of June 1^st of the new school year
2. A parent permit form (which is located on the backside of the physical form)
3. Must have attended a liability meeting and signed off on the paperwork
4. Spring Liability Meetings:

Liability meeting for winter sports will take place Nov 1^st, 6:00pm at the Woodbury Library.
